The following forms and instructions are in Adobe Acrobat "PDF" format. The forms work best with newer versions of the free Adobe Reader software. To be used by a recipient committee if the committee:
1- Is not controlled by a candidate;
2- Has not received a contribution which must be itemized;
3- Has not received any other payment of $100 or more;
4- Has no outstanding loands made or received; and
5- Has no accrued expenses.
Form 460 Recipient Committee Campaign Statement
This statement is used to disclose receipts and expenditures as well as the filer's total financial activity for various periodic reports. This form is also used as an amendment to update missing or incorrect information.

The paper statement is used to disclose independent expenditures of $1,000 or more to support or oppose a single candidate or a single measure in the jurisdiction of the election. Electronic filers, who make independent expenditures of $1,000 or more to support or oppose a state candidate or a state measure during the 90 days before an election, are not subject to this paper requirement.

You may use this form to report a late payment received by your slate mailer organization, or you may use another format, as long as the information requested on this form is provided. Do not use Form 497 (Late Contribution Report).

A Candidate for state or local office must file this form prior to solicitation or receipt of any contribution, or expenditure of any personal funds used for the election.

A committee that makes expenditures totaling $5,000 or more to an individual for his or her appearance in a printed, televised, or radio advertisement, or in a telephone message, to support or oppose the qualification, passage, or defeat of a state or local ballot measure.
Form E-530 Electronic Issue Advocacy Report (No official name)
E-530 Access to Secretary of State Site
This online only form (no paper requirement) is used to disclose ads or communications, which promote or criticize a state candidate but do not expressly advocate the election or defeat of that candidate. This form is only filed in connection with a candidate for elective state or statewide office. This form is also used to disclose payments of $5,000 or more received by the filer for such a communication.
